Support fragment comments

Description
Allow comments on specific fragments (text selections, blocks, or elements). There is a comment icon right now, but it doesnt embed the actual fragment, and the text is not correctly highlighted.

Expected Behavior
Users can attach comments to precise parts of content.

High priority usability improvement.
