I get duplicate notifications in two different situations, and we should de-duplicate them on the agent server:

When a comment is edited, there are extra notifications for each edit. I should only get one notif per comment. If I was notified already, it shouldn't get happen again. If I was not notified from the first comment but I am notified for the edit, then I should get the 2nd notification. For example when I was mentioned in an edit but not in the original.

When a comment is mentioning me and I am following a certain document (because I was an author on it), then I get a "Bob started a discussion on X" and also "Bob mentioned you in X" but really its triggered by the same comment so I should only see the mention notif.
